George Hoffman's Application for Admission to JCRS, 1910 November 12
Application form of George Hoffman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 21 at the time of the application. He was born in New York City. He also lived in New York City when he contracted tuberculosis. He had been sick for three years upon his arrival to Denver, Colorado. His occupation states he worked as a clerk and a law student. The verso of the application states he was admitted on November 18, 1910 as an emergency patient, and died on July 2, 1911.
