Hyman Weiner's application for admittance to the JCRS. It includes such information as age (40), place of birth (Russia), and occupation (shoemaker). He was married and had five children, had no means of support, and his nearest relation was his wife, Rachel Weiner, in Atlanta, Georgia. On the backside, it is written that he had tuberculosis in both lungs, was admitted on March 4, 1909 and discharged on August 10, 1909.
