Abstract
Application form of Hyman Daily for admission as a patient to the Jewish Consumptives' Relief Society. He was age 16 at the time of the application. He was born in Russia and immigrated to the United States in 1909. He lived in Houston, Texas when he contracted tuberculosis. He had been sick for six months upon his arrival to Denver, Colorado. His occupation states he worked as a school boy. The verso of the application states he was admitted on August 8, 1910 and was discharged on September 15, 1910.
