Abraham Kovalsky's Application for Admission to JCRS, 1911 April 6
Abstract
Application form of Abraham Kovalsky for admission as a patient to the Jewish Consumptives' Relief Society. He was age 23 at the time of the application. He was born in Russia and immigrated to the United States in 1905. He lived in Rochester, New York when he contracted tuberculosis. He had been sick for three years upon arrival to Denver, Colorado. He was single. He also worked as a tailor. He was in the Municipal Hospital of Rochester, New York before applying for JCRS. The verso of the application states he was admitted as an emergency case on May 2, 1911 and died on June 9, 1911.
