Application form of Joseph Bernstein for admission as a patient to the Jewish Consumptives' Relief Society. He was age 32 at the time of the application. He was born in Clebeland, Ohio. He lived in New York City when he contracted tuberculosis. He had been sick for one year upon arrival to Denver, Colorado. He was married, but did not have any children. He also worked as a salesman. The verso of the application states he was admitted on April 12, 1911 and was discharged on October 19, 1911.
