Morris Feldman's Application for Admission to JCRS, 1911 April 11

Application form of Morris Feldman for admission as a patient to the Jewish Consumptives' Relief Society. He was age 26 at the time of the application. He was born in Russia and immigrated to the United States in 1907. He lived in St. Louis when he contracted tuberculosis. He had been sick for seven weeks upon arrival to Denver, Colorado. He also worked as an operator. The verso of the application has a section titled "Report of Medical Examiner," which states teh patient was suffering from stage two pulmonary tuberculosis and is quite ill. The center section of the verso states Feldman's application is an emergency case. The verso also has "Application No. 1787, Application of Morris Feldman, Address 1471 Knox Court, Denver, Colorado, Received April 11, 1911, Admitted June 10, 1911, and Died July 15, 1911.
