Application form of Samuel Goldfine for admission as a patient to the Jewish Consumptives' Relief Society. He was age 18 at the time of the application. He was born in the United States. He lived in New York City when he contracted tuberculosis. He had been sick for three years upon arrival to Denver, Colorado. He also worked as a bookkeeper. He was in the National Jewish Hospital and Ottisville Sanatorium before applying for JCRS. The verso of the application has a section titled "Report of Medical Examiner," which states the applicant was in stage three pulonary tuberculosis. The center section of the verso has "Application No. 1794, Application of Samuel Goldfine, Address 1521 Julian Street, Denver, Colorado, Received April 21, 1911, Admitted May 26, 1911, and Died January 8, 1912."
