Application for admittance to JCRS filled out by hand for Philip Kahan and dated July 15, 1907. Information on the form includes his age, birth place, occupation, residence, marital status, nearest relatives, and signed Philip Kahan. The back of the form has sections titled "Report of Medical Examiner" that says: "July 29, 1907, I have examined applicant and do find him suffering from pulmonary tuberculosis. Stage Remarks Mr. P. Kahan entered alone date as emergency patient suffering from tuberculosis and for general [illegible] word." and "Admission and Dismission Committee," is left blank. The middle section of the back is a form filled out by hand. It reads, "Application No. 532, Case No. 338, Application of, Philip Kahan, Address, Received July 15, 1907, Accepted June 29, 1907, Rejected, Admitted June 29 1907, Readmitted, Discharged, Left April 24 1907, Died, remarks emergency."
