Overview

Application for admittance to JCRS filled out by hand for Gus Hornstein and dated July 15, 1907. Information on the form includes his age, birth place, occupation, residence, marital status, nearest relatives, and signed Gus Hornstein. The back of the form has sections titled "Report of Medical Examiner" that says: "July 4, 1907, I have examined applicant and do find him suffering from pulmonary tuberculosis. Stage Remarks Gus Hornstein is next in by Dr. C. D. Spivak as emergency case, on account of general weakness" and "Admission and Dismission Committee," is left blank. The middle section of the back is a form filled out by hand. It reads, "Application No. 533, Case No. 341, "Second" Application of, GUs Hornstein, Address, Received July 15 1907, Accepted July 4, 1907, Rejected, Admitted July 4 1907, Readmitted, Discharged, Left July 16 1907, Died, remarks 'Emergency'."
