Application for admittance to JCRS filled out by hand for Yetta Ginsberg and dated July 17, 1907. Information on the form includes her age, birth place, occupation, residence, marital status, nearest relatives, and signed Yetta Ginsberg. The back of the form has sections titled "Report of Medical Examiner" that says: "Date July 17 1907, I have examined applicant and do find him suffering from pulmonary tuberculosis. Stage Remarks Mrs. Yetta Ginsberg entered the Sanatorium [illegible] word, (by ambulance) suffering from acute tuberculosis and in extremely bad condition. She was [illegible] and died during the night. M. R. Smirnow" and "Admission and Dismission Committee," is left blank. The middle section of the back is a form filled out by hand. It reads, "Application No. 535, Case No. 347, Application of, Yetta Ginsberg, Address 2635 Arapahoe St., Received July 17 1907, Accepted, Rejected, Admitted July 17 1907, Readmitted, Discharged, Left, Died July 17 1907, remarks 'Emergency', Buried in Mount Nebo Cemetery 7/18/07 Sec. L. Lot 58 1/2."
