Abstract

Application for admittance to JCRS filled out by hand for Morris Strymber and dated July 20, 1905. Information on the form includes his age, birth place, occupation, residence, marital status, nearest relatives, and signed Edward Lipkin. The back of the form has sections titled "Report of Medical Advisory Board," blank, and "Report of Executive Committee," blank. The middle section of the back is a form filled out by hand in ink. It reads, "No. 111, 62 [both crossed out] 111, Case #70, Application of Morris Strymberg, July 20/05, Date of Admission Sept. 19, 1905, Died January 13, 1906.

Inscription and Marks

Handwritten at the top of the application, "His wife is in insane asylum. Mrs Feinberg called and told a pitiful tale. 9-19-05." Handwritten in the upper righthand corner, "Rec'd 7/20/05, all, 9/19-05." Stamped just below this inscription in blue ink, and filled out in black ink, "Received July 20 - 05, Admitted Sept. 19-05, Discharged, Left Male, Died Jan. 13, 06." Handwritten at the bottom of the application, "Died Jan. 13."
