Abstract

Application for admittance to JCRS filled out by hand for Wolff Woldman and dated Oct. 6/05. Information on the form includes his age, birth place, occupation, residence, marital status, nearest relatives, and signed by Wolff Woldman. The back of the form has sections titled "Report of Medical Advisory Board," blank, and "Report of Executive Committee," blank. The middle section of the back is a form filled out by hand. It reads, "No. 140, Case #74, Application of Wolff Woldman. Date of Admission, Oct. 6/05, Date of Admission October 13, 1905. Left April 16, 1906.

Inscription and Marks

Handwritten at the upper right-hand corner, "Rec'd Oct. 6, 1905." Handwritten at the bottom near the signature line, "Attach to this the former letter from Guttman & my [illegible]. Admitted." A form stamped in blue ink across the upper right of the document is filled out in the following manner, "Received: Oct 6 - 05, Admitted: 10-13-05, Left: April 16/06."
