Application of Benjamin Feinman to JCRS, 1905 October 11
Abstract
Application for admittance to JCRS filled out by hand for Benjamin Feinman and dated Oct. 11/05. Information on the form includes his age, birth place, occupation, residence, marital status, nearest relatives, and signed by B. Feinman. The back of the form has sections titled "Report of Medical Advisory Board," blank, and "Report of Executive Committee," blank. The middle section of the back is a form filled out by hand. It reads, "No. 142, Case #79, Application of Benjamin Feinman. Oct. 11/05, Date of Admission November 10, 1905. Left October 29, 1906. Handwritten below, "Doing fairly well (July 06)."

Handwritten at the upper righthand corner, "Received Oct. 11, 1905." Handwritten at the bottom near the signature line, "Accept, admitted Oct." A small form stamped in blue ink at the upper right of the document is filled out, "Received: Oct. 11/05; Accepted: Nov 1, 05; Admitted Nov. 10th."
