Abstract

Investigation Blank of United Jewish Charities of St. Louis filled out for Gabriel Bernhard, dated March 1, 1910. The top part of the form includes blanks for date, cause, surname, nativity, arrived in U.S., arrived in city, from, length of residence in former city, residence, former residence, floor, rooms, monthly rent, societies or lodges, applies for. The second part of the form is a grid with spaces for given name, age, social state, occupation, where employed, and weekly income when at work. A long note is scrawled across the bottom of two of these blanks noting some of the history of Gabriel Bernhard and his last known whereabouts.
