Abstract

Application for admittance to JCRS filled out by hand for Lottie Laude and is dated May 1906. Information on the form includes her gender and is not signed. . The back of the form has sections titled "Report of Medical Advisory Board" and "Report of Executive Committee," both are blank. The middle section of the back is a form filled out by hand. It reads, "No. 239, Case #144 (crossed out) 143, Application of Lottie Laude, Date of Admission, May 23 1906, Date of Discharge (crossed out) Death, June 2 1906, Emergency. Buried in Mount Nebo Cemetery 6/6/07 Sec. L. Lot 51. (handwritten)."
