On the backside it reads he was admitted on February 14, 1912, and died on April 02, 1913; "Buried at Golden Hill No. 347, Blk. 4."

He was singl and his nearest relatives were his mother and brother in Ohio. There is a post it note stuck the front page that reads "Father belongs to Jishrum Lodge #140 I.O.S.B." On the backside it reads he was admitted on 1907 June 03, and discharged on 1908 February 06.
