Handwritten application for admittance to the JCRS in pen for Hyman Osherofsky, with information such as age (19), place of birth (Russia), and ocupation (Operator). His nearest relative is his father, in N.Y.C. The form is signed by H. Osherofsky. On the the back side it is written he had stage III tuberculosis, advanced case, both lungs involved.
